Final Forms

The HS staff is urging all parents and students to complete their online Final Forms. Included in these forms is the Emergency Medical Contact document. This form gives us accurate information about your child’s medical needs as well as up-to-date contact numbers in case we need to get a hold of you. This information is critical for us to have while your child is in our building. Please complete these forms today!
